If a medicine is out of your body in say 4 hours, can side effects from
the drug still exists?

I took viagra a few times a year ago and the next day it felt like I
had "attacks" where I had to make myself breathe. I understand this
medicine is out of your systemin 4 hours. How then could it cause this?
Doctor could not find anythign wrong with my heart.

Viagra has a half-life of 4 hours.  That does not mean it is completly out
of your system in 4 hours.

How long does it take to get out of your system then?

It will vary with the person, but on average, half should be out in four
hours, half of what's left should be out in the next four hours and so
forth.
