Is fibroadenoma a disease or a one time thing

Is fibroadenoma a disease, a one time thing, or continous thing such
that once you have it, you will have it again.

If a surgery has been performed (and biopsy done after the suregry) and
the lump removed is firboadenoma, is it considered as pre-existing
condition?
