  Depends on the business. Some take it out and send it weekly,
some monthly, some bi-weekly, etc. If you want to pay for it
yourself, you can keep it up 18 months.

And then you have ONLY 30 days to convert to a private plan where you have
to be accepted.  Of you wait too long, you have to make it through
underwriting so if you are uninsurable or insurable but with high high
premiums, go for the covertion and don't let it lapse.
