crackles??

I posted to asthma gp but it is not a very active group, I thought I
might get some help here.  Thanks

I was seen by my lung Dr on Friday.  I have had increasing coughing
5-10 times a day even though I am using advair 250, QVAR80 4x a day,
rhinocort, and albuterol to try to stop the coughing.  I had a pulse
oximetery done with a start of 92% going up to 97%.  When I did the
resting part I went down to 89% then 90% for about 7 minutes then
gradully got up to having 2 97% after about 12 minutes.  When I saw the

dr he asked when I had my last chest xray and it was in Jan.  I had a
very bad chest cold and was out of work for 2 weeks with wheezing.

this new coughing started about a month ago, I don't cough up anything,

just this very wet cough, as if I had a cold but no cold.

He said that he heard crackles and he wanted me to have a chest xray.
Why would I need an xray why not an antibiotic?  He also added spiria.
I am getting more air but still have a wet cough.

I have been seeing my Dr for about 10 years and he has never mentioned
crackles, I am just curious as to what could be going on.

"Crackles" is just a term like many others to describe adventitious
sounds heard in the lungs. Like many terms in medicine, it may mean
different things to different people.

Crackles are typically regarded as different than wheezing. Crackles
can best be mimicked by rubbing your a small clump of your hair
between two fingers right next to your ear (if your hair is long
enough). While not entirely specific, "fine" crackles tend to indicate
fluid in the alveoli whereas "coarse" crackles may result from mucus
or fluid in the airways. Crackles are typically heard on inspiration.

Wheezing, by contrast, is typically thought of as polyphonic, musical
sounds. That is, one hears many sounds simultaneously that have an
actual "note", as opposed to the staccato white noise that makes
crackles. Wheezing is the usual sound heard from bronchospasm. Also,
unlike crackles, wheezing is typically heard on expiration.

Crackles may be from pneumonia, but other lung conditions can cause
crackles as well. An X-ray can help differentiate these to determine a
proper diagnosis and thus proper treatment. The answer to "why not an
antibiotic?" is because it might not be a bacterial infection.

Thanks for the information, I had my chest xray and I have to check in
Sunday with my Dr maybe he will have some information then.  Your
right, I jumped the gun about antibiotics, my dr is very conservative
about them he never prescribes the same one twice in a row so he sure
wouldn't just prescribe for no reason.
