New and troubled coumadin patient ;-)

I sure hope someone can shed some light for me.  I'm a 43 year old
woman that experienced a pulmunary embolism 12/30/04 and was put on 15
mg. coumadin daily (had troubles getting INR into theraputic range - 15
seemed to be the magic number).
 Am suddenly experiencing major fluctuations with INR.  No changes in
diet, etc.  In March, INR jumped to 3.4; April, it dropped to 1.1, 1.4
and 1.2 over 2 week period.  Dr. actually suspected the lab of errors,
but no such luck.  Am now taking 18.25 mg daily and am awaiting next
blood draw (5/17)to see if things have stablized. I was originally told
I would only be on warfarin until July.  Now I seriously wonder.
  No DVT's were ever found; no one knows where the clot came from
and/or why I threw it.  ALL blood tests came back clean, including
tests to rule out Hugh's Syndrome and antiphosphoruslipid syndrome (?)
- both of which my mother has.

Question 1 - How dangerous is it being on such a high dosage?  I am
more than a little nervous...

Questions 2 - Any hypothications as to what in the world is happening
to me?  My dr.'s are passive, to say the least (HMO)

Lastly,
Question 3 - Can there be biological false negatives to the tests ran?
I have many of the symptoms of Hugh's, including chronic migraine.

Am more than grateful.
