I have a friend with neuropathy, it doesn't appear to be any of the
usual suspects (diabetes, etc.), he has been to a number of doctors.
The neuropathy has come slowly over 30 years starting in his late 30s.
It only appears to significantly effect sensation in the feet, and
possibly lower legs.
Can anyone suggest any conditions consistent with this that might be
responsible? Are there any treatments that should be tried even with
an unknown cause?

EMS is a well proven treatment for neuropathic pain. Pain clinics use this
sort of technology.

I would favour ruling some things out.

There a a large number of conditions which can be associated with
neuropathy. The cause can generally be determined by a set of
studies. I would recommend consulting a good academic-type
neurologist, probably at a major medical school.
