My patient had pancytopenia
despite our platelet transfusion
his platelet still around 7K
We are afraid of spontaneously bleeding
please tell me how to treat this patient
Thanks

If the patient has antibodies that are destroying donor platelets, you
could try a continuous a platelet drip (one unit over 4 hours, that
is, 6 units per 24 hours), or try to obtain HLA-matched platelets.

Are you sure the patient has aplastic anemia? Has he had prior platelet
transfusions? It would be surprising not to get a bump in the platelet
count with a first platelet transfusion in someone with aplastic anemia.
After the patient has had multiple transfusions, though, he can develop
antibodies and may not respond well to platelet transfusions.
