What could be causing pain in abdomen?

Young 16 yr. old friend of mine cannot get to see a doctor (her family
is awful and irresponsible) she has complained of recurring pain in
the abdomen approximately two inches to the right of her navel. Pain
lasts for hours to days at a time, during which she cannot stand or
sit up without leaning forward. Recurs approximately bi-monthly - once
or twice every 3 weeks. So severe she cannot stand up, or sit up
straight - the whole time. She claims to not have other symptoms with
the pain. We've asked about fever, constipation, diarrea, dizziness
all of which she denies. This has apparently been happening for years
and has not changed in its intensity or frequency, but its duration
varies. She has never seen a doctor for this condition. She says that
putting her knees to her chest actually helps the pain rather than
increasing it. She does not have any apparent protrusions in that
area. We've advised her to go talk to a doctor or a nurse first thing
in the morning, but do not know if she will or will be allowed to. We
would take her to see a doctor ourselves, but she is thousands of
miles away from us.

Any ideas on what this could be? We've thought it could be gas, or
gastro-intenstinal infection, appendicitis, or hernia, but are not
getting any clear indications of any of them.

If it were her appendix, I don't think it would last for years first
of all.  Gas usually occurs in the center and can go a bit to the
right if it's bad enough.  Getting her Maalox couldn't hurt her to
just see if it's gas.  Sometimes (I work in an ER and see this a lot.)
it can be emotional issues.  Women tend to be famous for complaining
of abdominal pain if they're having any issues in their life that are
bothering them.  There are so many things that can cause abdominal
pain.  Without the proper work-up with a doctor, it's hard to say what
it is.  It could even be issues with her cervix etc.  So many
things.....

I don't know where your friend is but there are usually clinics that
will see patients without charging them.  I'm not sure how the
parental consent issue will work though.  You might want to check with
a medical clinic in your area just for curiousity sake.  But if she's
unwilling to even attempt to go, there's not much you can do sadly.  I
wish you luck.

the lack of any other associated symptoms and the lack of change "for years"
and the fact that it never got severe enough for her to be hospitalized or
see a doctor is reassuring.

she should see a doctor, however, as soon as possible.  i find it hard to
believe that a 16-year old child's family would refuse to get her medical
attention.  if you're really concerned, notify the authorities -- this is
child abuse.

I fully agree with you Beachhouse.  If I were a witness to such
neglect, I'd report it as well.  Abdominal pain can be caused by many
things and without a doctor, there's not much that can be done fo
rthis child.  Report the parents!
