The 14th amendment and equality have nothing to do with the cited
article.  Nor does the article substantiate anything about women not
being equal (much less "created equal") - no rate is reported for
overweight or diabetes for any other group of women, nor is evidence
cited that the women are overweight because of genetics; indeed the
article points out cultural reasons for obesity and the doctor is
trying to change those statistics and attitudes, which would not be
possible if they were "created unequal".

In part, it is both.  Depends on what is being treated, and how.
