Does anyone know where one can download a copy of this study?  I
downloaded it several years ago, but it seems to have been removed
from the web, and the agency itself was seemingly absorbed into
another agency, which then disappeared.

This was an exceptionally useful and sane overview of back problem
therapies, notable for its measured scepticism about most treatments.
I would like a copy to send to a friend.
