Cause of Acute Myocardial Infarction

For a healthy active old man with no diabetes and cardiovascular
problem and NO any other medical history.

Few days after an uneventful leg operation and his leg wound is
recovering with the VAC therapy system, he had an AMI and pushed to
ICU, and the following biochemical test results showed his Hb is 4.20.

What can be the likely cause for his AMI?

hmmm...interesting case...firstly I would wonder if the AMI was due to a
blood clot thrown from the leg operation???

the very low Hb sorta confuses the issue...unless he bled a LOT his Hb is
unlikely to get that low...certainly not without symptoms
ie..fainting..issue...

so you know his other chems?? such as tropinin?? which is done for all who
have had AMI

me thinks the very low Hg is the culprit..

again..let us know..

and hope he is doing better now

Few hours before the attacked, he has rapid pulses, low BP, pale,
breathlessness, semiconcious, and was given IV NaCl.

Biochems done in the ICU showed: Hb=4.2, heamactocrit 14.8, AST=1221,
ALT= 631, LDH= 58, CK=1370, CKMB=101,
troponin= 11.75

Kind of hypovolemic shock?

He survives with: initially, complete renal failure, pulmonary edema,
pleural effusion. Thank god, all returned to normal after 3 weeks.

Echocardiography revealed his left heart chambers dilated, mild aortic
& mitral valves regurgitation, regional wall motion abnormalities and
left ventricular systolic dysfunction.

Will his cardial function recover to normal?
