Medication Errors

Let us talk about a very important subject that everyone might
experience, this subject is "MEDICATION ERRORS" which might occur by
physicians, pharmacists or nurses. I believe that any patient at any
time could be a victim of a medication error due to his/her doctor,
pharmacist or nurse. This is scary! How we can protect our selves from
such errors that sometime could be fatal and lead to death.

The studies show that 90,000 people die every year in U.S.A because of
medication errors. This number is really frightening. But what about
the countries were there is no or weak reporting system and no one
knows how many medication errors occurred and what the results of such
errors were. How we can reserve the patient's right in knowing the
medication errors that occurred to him/her. Moreover, how the patient
should be compensated for the damage that occurred to him/her.
I believe that medication errors are recently increasing despite of
using and implementing more advanced technologies and precautions.

So, how we can protect our selves from being a victim of such
medication errors and what is the role of the health care providers in
minimizing or preventing medication errors?

I do agree that medication errors are a frightening matter, but i
believe that ALL health care providers ( including radiologists and
physiotherapists, etc...) are responsible to prevent, detect and
report these errors. in addition, patients and patient's care givers
play an impotant role and could be the cause of error. Our aim in
health care is to spread the culture of prevention and to educate the
ways and the importance of reporting these errors. This is a
continuous task that should be followed by permanent work forces,
which coud, thereafter, start collecting data for statistical purposes
and for preventative measures.
Education is the key.
