I went most of this year with jock itch that I didn't do much to
treat.
Finally went to the doctor six weeks ago and was given a many-refill
prescription for Nystatin. I've been using it faithfully ever since,
twice a day, rubbing it on my inner thighs and in my rear end crack.
The doctor had told me that, considering how long I'd let the jock
itch persist, it might take a couple months to treat. And honestly,
things seemed to improve - not much, but a bit - for a while.
But then last week, it got really bad. The whole underside of my
scrotum just itches like crazy. I'd been able to resist scratching
during most of my time on Nystatin, but now I just want to scratch it
silly. I actually feel some kind of "rush" feeling when I do that,
like it's releasing a drug into my system. I remind myself of a dog.
The whole area (inside of legs, underside of scrotum, rear end crack)
feels moist all the time. It smells odd too, even in the shower when
I'm soaping those areas up.
Could I have something worse than jock itch? If so, what? Are there
more aggressive treatments for the symptoms I'm showing? It actually
hurts a bit now to walk or sit.

I think you need to share this with your doctor, not us.
We can't examine you, don't want to examine and did not prescribe the
nystatin.

Not an expert on jock itch, but I've heard of women with vaginal
yeast overgrowths that got to that state, so it CAN happen. Fungus
is fungus.

Have you tried alcohol? I won't try to tell you it's fun, but on
the plus side, it will stop the itch for longer and longer with each
treatment. Your object is to keep the area as dry as possible. At
least 2-3 times a day, wipe it down with rubbing alcohol, let air
dry, then dust with some absorbent powder like Gold Bond. Try to
minimize sweating, or be sure to treat directly after. I'd be
surprised if it lasts more than a week with that treatment. Once
you think it's gone, keep up the treatment for another 3 days. This
also works on athletes' foot, fwiw.
A high sugar diet can make it harder to get rid of, and might be why
it's flaring up rather than receding now. Cut way down on the sugar
and white flour. Antibiotics and antibacterial soap can make it
flare up because they kill the natural skin bacteria that is the
fungus' competition. If your wife/girlfriend has a yeast infection
your problem will likely keep coming back. In your spot I'd also
replace underwear and towels/washcloths once it stops oozing and
feeling slimy.

Saw my doctor the other day and he reconfirmed that it's jock itch,
but took me off Nystatin and got me a prescription for clotrimazole.
Wow, in less than 24 hours, most of the itching is gone. The doctor
said that the rash itself should be gone within a month. Which is
great, but for now I'm just happy to be rid of the itching.
