Capsaicin Patch Relieves Painful HIV-Associated Neuropathy
NEW YORK (Reuters Health) Jun 23 - In a double-blind randomized study, a single application of a high-concentration capsaicin skin patch (NGX-4010) produced a sustained reduction in pain over 3 months in patients with HIV-associated distal sensory polyneuropathy, the most common neurologic complication of HIV infection.
"NGX-4010 could provide a promising new treatment for painful HIV neuropathy," Dr. David M. Simpson, from Mount Sinai School of Medicine in New York, and colleagues say in the June 10 issue of Neurology.
Following application of a topical anesthetic, 307 patients with painful HIV-associated distal sensory neuropathy applied a single NGX-4010 or control patch for 30, 60, or 90 minutes to painful areas on the feet.
After a transient treatment-related pain increase, the capsaicin patch resulted in a mean pain reduction of 22.8% during weeks 2 to 12 compared to a mean pain reduction of 10.7% in the control arm (p = 0.0026), the investigators report. "Significant improvement was apparent by week 2 and continued throughout the controlled 12-week observation period," they write.
Mean pain reduction in the 30-, 60-, and 90-minute application groups was 27.7%, 15.9%, and 24.7%, respectively. One-third of NGX-4010- treated patients reported a 30% or greater drop in pain from baseline compared with 18% of control patients (p = 0.0092).
Self-limited, mild-to-moderate local skin reactions were common but "manageable," Dr. Simpson and colleagues report.
"Data from the current study support the use of NGX-4010 both in combination with systemically acting analgesics and as monotherapy," they conclude.
NGX-4010 is being developed by NeurogesX, Inc of San Mateo, California.

Research demonstrates that antioxidants and metal chelators may be of beneficial use in the treatment of neurodegenerative diseases, such as Alzheimer's disease (AD). This study investigated the antioxidant and metal-binding properties of curcumin, capsaicin, and S-allylcysteine, which are major components found in commonly used dietary spice ingredients turmeric, chilli, and garlic, respectively. The DPPH assay demonstrates that these compounds readily scavenge free radicals. These compounds significantly curtail iron- (Fe (2+)) and quinolinic acid (QA)-induced lipid peroxidation and potently scavenge the superoxide anion generated by 1 mM cyanide in rat brain homogenate. The ferrozine assay was used to measure the extent of Fe (2+) chelation, and electrochemistry was employed to measure the Fe (3+) binding activity of curcumin, capsaicin, and S-allylcysteine. Both assays demonstrate that these compounds bind Fe (2+) and Fe (3+) and prevent the redox cycling of iron, suggesting that this may be an additional method through which these agents reduce Fe (2+)-induced lipid peroxidation. This study demonstrates the antioxidant and metal- binding properties of these spice ingredients, and it is hereby postulate that these compounds have important implications in the prevention or treatment of neurodegenerative diseases such as AD.

They are now **targeting** iron as a **SPECIFIC** pain reliever. This study of a 'patch' containing an iron chelator BEING.. soooo .. good .. would give credence to my hypothesis.

Vegetable oils have been shown to have their 'properties' pass through the skin into the body when they are applied to the skin .. slathered .. on. This would allow the phytic acid / phytate IN the vegetable lecithin to bind up the iron in the body. Phytic acid applied to the skin has been shown to raise the phytic acid levels in the body. Phytate is the substance drug companies 'hold up to the light' WHEN they attempt to make iron chelators.
"Does it work as good as phytate?"
They've shown adverse effects with synthetic iron chelators.

Background:Soy protein or its components may protect against the atherosclerotic cardiovascular disease (CVD) risk factors total homocysteine (tHcy), C-reactive protein (CRP), and excess body iron, which generally increase with menopause.
Objective:The primary objective of this study was to determine the independent effect of the soy protein components isoflavones and phytate on CVD risk factors in postmenopausal women. The secondary objective was to identify factors [blood lipids, oxidative stress indexes, serum ferritin, plasma folate, plasma vitamin B-12, and body mass index (BMI)] contributing to tHcy and CRP concentrations.
Design:In a double-blind, 6-wk study, 55 postmenopausal women aged 47– 72 y were randomly assigned to 1 of 4 soy protein (40 g/d) isolate treatments: native phytate and native isoflavone (n = 14), native phytate and low isoflavone (n = 13), low phytate and native isoflavone (n = 14), or low phytate and low isoflavone (n = 14). We measured iron indexes, tHcy, CRP, and BMI.
Results:Soy protein with native phytate significantly reduced tHcy (P = 0.017), transferrin saturation (P = 0.027), and ferritin (P = 0.029), whereas soy protein with native isoflavones had no effect on any variables. At baseline, BMI was highly correlated with tHcy (r = 0.39, P = 0.003) and CRP (r = 0.55, P < 0.0001), whereas HDL cholesterol was correlated with CRP (r = –0.30, P = 0.02). Multiple regression analysis showed that LDL cholesterol and BMI contributed significantly (R2 = 19.9%, P = 0.003) to the overall variance in tHcy.
Conclusion:Consuming phytate-rich foods and maintaining a healthy weight may reduce atherosclerotic CVD risk factors in postmenopausal women.

Chorea is a hyperkinetic movement disorder characterised by excessive spontaneous movements that are irregularly timed, randomly distributed and abrupt. In this article, the authors discuss the causes of chorea, particularly Huntington's disease and the genetic syndromes that may resemble it, including HDL1-3, inherited prion disease, spinocerebellar ataxias 1, 3 and 17, neuroacanthocytosis, dentatorubro- pallidoluysian atrophy (DRPLA), brain iron accumulation disorders, Wilson's disease, benign hereditary chorea, Friedreich's ataxia and mitochondrial disease. Acquired causes of chorea include vascular disease, post-infective autoimmune central nervous system disorders (PANDAS), drugs, systemic lupus erythematosus, antiphospholipid syndrome, thyrotoxicosis, AIDS, chorea gravidarum, and polycythaemia rubra vera. The authors suggest an approach to the clinical assessment of chorea, the value of investigations, including genetic tests (for which they offer a structured framework highlighting the importance of prior counselling), and finally briefly discuss symptomatic drug treatment of chorea.

Trehalose opens new approach to treatment of polyglutamine diseases
Huntington's disease (HD) is a progressive neurodegenerative disorder caused by a mutation that expands the CAG polyglutamine repeat in the huntingtin gene beyond normal. As the formation of insoluble protein aggregates may be linked to the cellular distortions underlying HD, molecules or proteins that inhibit their formation have been investigated at RIKEN. In vitro screening studies showed that various disaccharides can inhibit polyglutamine-mediated protein aggregation. The most potent compound, trehalose, stabilized proteins containing an expanded polyglutamine and decreased formation of aggregates dose- dependently without inducing any cellular toxicity. Oral administration (2%) in the R6/2 transgenic mouse model of HD decreased polyglutamine aggregates in the cerebrum and liver, improved motor dysfunction and extended life-span in a dose-dependent manner. The elevated rate of diabetes in R6/2 mice mimics symptoms observed in HD patients. The examination of fasting blood glucose in R6/2 and wild- type trehalose-treated (2%) mice were comparable to that of control mice. The interaction between disaccharide compounds and polyglutamine- mediated protein aggregation may provide a novel approach for the treatment of polyglutamine diseases (Tanaka, M. et al. Nat Med 2004, 10(2): 148).

PROPERTIES OF TREHALOSE Trehalose is a disaccharide composed of two glucose molecules bound by an alpha, alpha-1, 1 linkage. Since the reducing end of a glucosyl residue is connected with the other, trehalose has no reducingpower. Trehalose is widely distributed in nature. It is known to be one of the sources of energy in mostliving organisms and can be found in many organisms, including bacteria, fungi, insects, plants, and invertebrates. Mushrooms contain up to 10- 25 % trehalose by dry weight. Furthermore, trehalose pro-tects organisms against various stresses, such as dryness, freezing, and osmopressure. In the case of resurrection plants, which can live in a dry state, when the water dries up, the plants dry up too. However,they can successfully revive when placed in water. The anhydrobitic organisms are able to tolerate the lack of water owing to their ability to synthesize large quantities of trehalose, and the trehalose plays a key role in stabilizing membranes and other macromolecular assemblies under extreme environmental conditions. The properties of trehalose are shown in Table 1. Its relative sweetness is 45 % of sucrose. Trehalose has high thermostability and a wide pH- stability range. Therefore, it is one of the most sta-ble saccharides. When 4 % trehalose solutions with 3.5 to 10 pH were heated at 100 °C for 24 h, nodegradation of trehalose was observed in any case. Because of nonreducing sugar, this saccharide does not show Maillard reaction with amino compounds such as amino acids or proteins. Its particular physical features make it an extremely attractive substance for industrial applications.
