This is just a printable at this time.  This way here you can print out
a new one as your info changes.  I don't know how to do a form type as
of yet.

On the first link I posted, there is a link on that site page for
medical alert tags, hope everyone got to see that site page and all the
tags I made.  I carry these tags with me in a key holder; I have latex
with the no symbol going through it for my latex allergy and I have
another tag on the other side.  

If anyone has a tag they want me to include let me know, I will see if I
can do it.

I just remembered, I use an address label for the top lines instead of
writing in the name and address info on the Medical ID printout.
