We are about to switch from slide agglutination to D-Dimers on a mini-vidas
(sp). Those of you doing automated D-Dimers: 1 What are you using? 2 Any
advise? Thanks

Personally I can't help but think that absolutely anything at all (including
random guessing) is better than the doubling dilutions & a range of 1000 -
2000.
We're doing pissy slide agglutinations at the moment !!!
Can't wait to automate it

We use the Sysmex CA 1500 for doing auto D-Dimers. We did clinical
correlation studies with the ER involving R/O PE. A positive predictive
value was calculated and is used in our reports as a reference range along
with the higher values normally associated with DIC.
The controls we originally used were the controls we use with normally on PT
and PTT. That didn't work too well so we switched to a more stable less
erratic dedicated low and high D Dimer controls.

We also have a CA-1500. I froze about 40 negative (via slide agglutination)
specimens in hopes of verifying the negative predictive values on the
CA-1500 curve. I could not get pasted the ultrasound or nuclear medicine
docs saying hipa Hipa HIpa HIPa HIPA. Our pathologist would not help so all
the reagents went to waste, and I am on to the mini vidas that does not have
the same FDA requirement for NPV.

The mini-vidas works well. Calibrations are simple enough, it is easy
to maintain. Controls are rock steady. I think you will like the change.

For those of you who are struggling with trying to obtain cutoff
values for the Sysmex CA1500. Our CAS sent me an email regarding FDA
approval of a cutoff of 1.0 for d-dimers testing on the 1500. Now I
can stop reviewing charts!!!

That is good news, but a little late for me.
