The serum one is used to evaluate renal function and the urine one is used
in evaluating other urine analytes in terms of a ratio between the two such
as protein/creat ratio. It eliminates the wide variance in concentration of
urine by standardizing it to a urine creatinine ratio.

The serum creatinine is a better index of glomerular filtration than the
actual measure of a 24 hour urine collection and measuring the urine
creatinine. Several reasons for that.
