Just returned from doctor while there went over my 24 hour urine test.
Have other health issues right now to really question doctor as she
said she would need to speak with some people as to my results.  Did
my best to remember what form said.  Think there were 4 or 5 values.
First 3 which were all 2 didget numbers and then I believe
metanephrines.  Can't remember what normal range was, but mine was 417
which if I remember right was about 200 or 300 over normal.  The total
for all of the tests were 1 under high normal.
Need input PLEASE to understand what this may mean. Have had spiking
BP, sweats, depression and major pain attributed to bad back.

Do ask again PLEASE give me some insite as to what this may mean, ?
further tests, and prognosis.  Another reason I am asking is that we
live in rural Arkansas and I need to get a feel of whether I need to
get myself to a larger city with specialists.

There are thousands of chemicals that can be tested in the human body either
in urine or in blood. There is no way for anyone on this group to even try
to guess what those other tests are on your 24 hour urine test. If you were
tested for metanephrines that would tell us that you are taking a drug and
this is a test to measure how much is in your system.

The metanephrines of both urine and plasma are better markers for
pheochromocytoma than catecholamines. Other tests on urine would be
creatinine, VMA, UCAT
Symptoms are hypertension, palpitations, severe headaches, and sweating. It
can be very dangerous so I would get to a larger city with specialists.
Surgery is available if that is what you have.
Unfortunately the sensitivity and specificity of the UMET tests is dependent
on methodology, colorimetric vs HPLC.
Primary hypertension can also elevate UMET  so it depends on the elevation
and type of method that was used that provides the specificity.
Sensitivity is better and specificity is worse for UMET by HPLC compared to
colorimetric methods.
No one can diagnose you here and I certainly am not but I would suggest you
see a specialist about this and not rely on the local doctor.

Thank you both for the info.  As to drug, yes, I have been on up to
100mg of oxycontin a day for back.  Am now down to 40mg a day.
Realize that there would be no one, but a doctor with all the info to
really give me the final answer, but in all my years in and out of
doctors offices and as a medical claims proccer I had never ran into
this before.  I do appreciate the advice that if there does seem to be
a continued concern here with my doctor I will ask to be referred to
specialist.  Am just below Branson, MO so Springfiled, MO, St Louis,
Little Rock, Fayetteville, AR KC and Tulsa would be my closest
choices.  Do miss my Mayo Clinic when we lived in Iowa.

Thank you both again.  If there is a different board that I should
post anymore questions please let me know.
