It's been a few years since I had an outbreak, but I think I am having
another one, even though the symptoms are very different from before.
Right now the symptoms are:
1) Tingling down the outside of my left leg
2) Painful skin on my scrotum
3) Painful pustules on the end of my tongue
Does this sound like an outbreak?
If so, why are the symptoms so different from the ones I had before?

The nature of how your body reacts to the virus can change over time. Sometimes
other factors are involved. For instance, I never had any nerve involvement
until I had to take three months of prednisone. After that, I developed
neuralgia as part of my outbreak.
Symptoms 1 and 2 on your list are pretty normal for herpes. The third is
something that I would not associate with an outbreak normally. They say that a
painful tongue can be a sign of B12 deficiency.
