Have you seen this article? It covers a broad range of HVB inoculation issues,
both successful and otherwise.

From this, I gather the answer*s* to your first question are: if after two
3-shot courses you do not develop anti-HBs, it is highly likely you are a
primary non-responder to immunization - and/or possible that you are already
infected (did you ever get tested for that possibility?) and that the SOC does
not recommend further attempts at immunization.
As for the other question, I'm not sure I saw a *definitive* question/answer
covering that scenario, but a logical conclusion matches your conjecture...
