I had a open gallbladder removal and the surgeon did a liver biopsy. Can
anyone help me with the results? I know that it is not good,but at least
it's not Cirrhosis yet. My labs are all normal but my platelets are slightly
low and my glucose is a little high.

Liver,Needle Biopsy:

Chronic Hepatitis with mild activity.
Pericellular and Periportal Fibrosis with bridging and occasional nodules.
No overt Cirrhosis
Moderate Steatosis.

Pathologist comments:
Consistent with chronic hepatitis due to hepatitis c with mild
activity(grade 2) and stage 3 fibrosis(Batts-Ludwig) I cannot rule out a
component of steatohepatitis.

Microscopic Description:
The liver biopsy is adequate and consists of cores of liver tissue with
moderate micro and macrovesicular steatosis involving 60% of hepatocytes.
Balloon degeneration is not prominent. There is portal expansion with a
lymphocytic infiltrate,lymphoid aggregates and focal piecemeal necrosis.
Plasma cells are not conspicuous. There is very mild patchy lobular
hepatitis consisting of lymphocytes and neutrophils within hepatic lobules.
Councilman bodies are not identified. Mallory hyaline is not identified.
Intranuclear glycogen vacuoles are seen. The trichrome stain shows
pericellular fibrosis and prominent periportal fibrosis with bridging and
focal nodule formation. No PAS positive globules are identified following
diastase digestion, No stainable iron  All controls stain appropriately.

At Stage 3 ("bridging fibrosis") you're one stage below overt cirrhosis, and
with an Activity level of 2, the disease is progressing. The Steatosis means
you also are blessed with a fatty liver.

Fortunately - and very much on the up side - the lack of PAS-positive,
diastase-resistant globules means the biopsy didn't identify any HCC
precursors.

The low platelet count and elevated glucose could be related to the condition
of your liver. That's something you need to review with the treating
physician.

If you don't mind my asking (and forgive me if you've posted this information
before - my memory ain't what it used to be ;-) but how "normal" are your
liver enzyme labs? And are you pre- or post-treatment for the HCV?

My liver enzymes are as follows.

AST- 34
ALT- 42

Platelet- 135
Non Fasting Glucose- 114

All other labs were normal.

I have done 3 rounds of tx.1st pegasys,only partial responder. Then
Pegintron,non-responder. Lastly,daily Infergen. I had a quick response and
stayed clear for 1 1/2 years,then breakthrough. I discontinued tx in Feb.
2007. I'm now waiting for something new.

I think its strange that your liver enzymes are normal when your liver
is in such bad shape.

I wonder if you would be a candidate for some kind of maintenance
while you wait. With Stage 3 you cant wait long.

I've brought this up and according to my doc,maintenance therapy is pretty
useless for most of us. We have talked about trying Donna Fanellis (Elmo's
NP) program with Amantidine added. But he tells me that the chance of it
working are very low. I see him again in October and I am going to strongly
suggest another round of tx with amantidine or at least a maintenance dose
of Infergen,daily if need be. I'm tired of sitting and waiting for liver
failure.

I hope you are actively trying to get into the teleprevir clinical
trials.

But people like you...simply amaze me. I'm in day 327 of my 72 week
Pegasys & Ribavirin ordeal, my first shot at tx, and it's been BY FAR
the hardest thing I've ever done, so to read stories such as
yours...you have my admiration, support, and more than a little awe. I
don't know if I could do what you've done. I'd like to think I
could...but I really don't know.
