need opinions (and some hope)

Hi all,
those of you who have been here since at least 2004 should someway
recall my story.
I'm male, have been infected with HCV for 8 years (since 1996) until I
started treatment in October 2004.
Pre-treatment labs revealed that I had genotype 4, a very low viral load
(3,500 IU/ml) and slightly elevated ALT (~ 69).
Viral load was undetectable at week 4, and LFTs went back in the range
since then. I stayed clear during all the treatment which I completed in
September 2005. Viral load was undetectable at the end of treatment, 3
months and 6 months after the end of treatment so doctor told me I
obtained SVR.
More than this, I had all the routine labs scheduled 1 year after the
end of treatment, in September 2006 (4 months ago) and, again, they
showed undetectable viral load, normal LFTs and everything else in the
range.

Now in December my partner and I had some GI symptoms, something like
the usual GI flu symptoms with mild stomach aches and diarrhea.
She had another episode of it at the beginning of the year and I had one
too just few days after.

Her doctor then decided to schedule lab tests for her and today we
discovered that her ALT was 118 at the time of the symptoms. The doctor
already scheduled HCV antibodies test for her.

No doubt I am already in panic.

I need your opinion: I know that sexual transmission is possible but
rare, although I've read something about a French study showing higher
sexual transmission rates for genotype 4 in HIV/HCV coinfected
homosexual men.

We are a monogamous, heterosexual couple.
Last time we checked for HIV was in 2002, when she was pregnant and we
were both negative. Since then we've not had any risky behavior.
The same goes for me about HCV: I had no risky behavior after diagnosis
and, most of all, after treatment.

In spite of this I am feeling worse than how I felt in 2004.
The idea of passing it to her makes me crazy.
I am feeling really bad.
What do you think about it? Is it possible given all the factors listed
above?

Thanks in advance,
jeeb.

Hi Jeeb,
Perhaps it could have been a bout of Hepatitis A. This virus is usually
transmitted by the oral/fecal route, such as contaminated food.(food
poisoning) It causes an acute form of hepatitis (inflammation of the
liver), does not have a chronic stage, and will not cause any permanent
damage to the liver. Most people don't have symptoms except what you
described. It may just be that.  I don't think after all this time with
her, that she would be infected.  Has she ever been checked or
immunized?
I have empathy for you,  Hopefully, you will know the results soon.

Jeeb, wait for the results before you start making yourself sick. Is it
possible that she has been infected all along and just now starting to
show signs? I know there is nothing that I can say to keep you from
worrying, but you have my support. Hope everything turns out OK.

I bet the odds are in favor of you having suffered a panic attack :-)

You've read all the testimonials here - I don't think I've ever read anyone
post that their spouse became HCV+ via sexual congress.

Is it possible she took too much Acetaminophen while being beaten up by the
flu (or maybe Norovirus?) and the high ALT is reflecting an aggravated liver?

Hang in there. Relief is just a test result away :-)

Cheers

In late December 2006, I contracted viral gastroenteritis (aka stomach flu).
I thought it was food poison, but then a story hit the news about this
season's higher than normal GI virus infections. The symtoms are diarrhea
and vomiting. They can last from one to ten days, and range from mild to
"how can I still be breathing?"

Thought you'd like to know.

Geeb,
Calm down and wait for the test results.  There are many things that can
cause a transient increase in ALT, including GI issues.
I would have a Hep A,B,C Evaluation Panel run.  If nothing shows, consider
what meds she's been taking for the GI issues ( esp. acetaminophen ).
There has been a near epidemic of Norovirus rolling across North America for
more than six months now.  GI symptoms can be severe.  If not already done,
this test should also be considered.

Well,
first of all thanks for your kind replies.

I've read all of them and all give a possible good explanation to the fact.
Most of all, it does feel good to see that most of you wouldn't relate
this to Hepatitis C and probably this was actually what I was looking
for, a different (not influenced) perspective.

Now I am actually trying and be more rational.
What I hope is that it has been that stomach flu to make the LFTs go up.
Yes she took a Paracetamol some days but, as far as I know, she never
overdosed.

She'll see the doctor next Monday.

Again, thanks you all.

Cheers,
jeeb.
