34 year old handsome white male, shoe size 10, 6 feet tall, LOL, Hep C

THE GOOD: after 4 weeks of treatment my viral load dropped from
3,074,450 to 237,645

Hold the medals, I'm not trying to sprain my elbow patting myself on
the back, just providing some feedback to those newbies like me
wondering if this treatment of Ribavirin 600 mg twice a day and a
healthy shot of Pegasys works - at least I hope this means it's working

THE BAD: side effects, fatigue and dry cough, (I swear I will have a
six pack from all the coughing I'm doing, my abdomen muscles are
getting a workout) or one day I'll cough up a lung

AND THE UGLY: after 5 weeks my white cell count dropped significantly,
so they gave me NEUPOGEN again a lovely shot - I do this once a week
also, in between the Pegasys as they supposedly counter act

with all the needles I feel like a Rock Star

If your whites weren't already in the toilet, I'd suggest you toss a
couple of weeks of daily Infergen into the mix, along with the Pegasys,
to realize a truly dramatic drop in your viral load.  Going
'undetectable' as early as possible in tx is the major determinant in
whether you're going to get a SVR.  Are you having another pcr done at 8
wks?  At the rate you're going, I'd expect a 2-log drop from baseline by
that point.  Keep after it!!

thanks elmo, however no habla tx too well, so could you please spell
out for me PCR and even SVR and this whole 2-log drop, and you won't
believe this but I did take calculus once, I guess another side effect
for me could be dumming me up, LOL

Some brief definitions:

PCR = Polymerase Chain Reaction.  It is the methodology used to
detect/measure DNA of organisms (like Hep C virus);  can be a quantitative
or qualitative assay.

SVR = Sustained Viral Response.  Lack of detectable virus in the blood after
tx is complete.  Usually defined as no detectable virus six months post tx.

2 log decrease = A reduction in viral load by a factor of 100 (10 x 10 = 2
log).  For example, viral load goes from 600,000 to 6,000.  This is the
minimal target goal for monitoring the effectiveness of tx at the 12 week
point.  The greater the log decrease, the less detectable virus in the
blood.

Your baseline was 3,074,450 which is about 6.5 log. (10 to the power of 6.5)
Just use your PC calculator to work it out. Calculus not needed.
After 4 weeks you have 237,645 which is about 5.3 log. The difference is 1.2
log, so you've had a 1.2 log drop by week four.

A 2 log drop by week 12 is required if you're to have a chance of getting an
SVR (sustained viral response - ie. no virus 6 months after end of tx). Elmo
thinks you'll get the famous 2 log drop by about week 8 at this rate.
