Summary

Chronic hepatitis C patients are advised not to share toothbrushes,
razors, nail-scissors or other personal articles that potentially may
have been in contact with blood, with others. This study examines the
contamination of toothbrushes in patients with chronic hepatitis C as a
model for a possible unconventional way of transmission. In 30 patients
with chronic hepatitis C, 2 mL of saliva (before and after
toothbrushing) and the toothbrush rinsing water after toothbrushing were
tested for HCV-RNA. Saliva before and after toothbrushing was positive
for HCV-RNA in nine (30%) and 11 patients (36.7%), respectively. Twelve
of the toothbrush rinsing water specimens (40%) tested HCV-RNA-positive.
In six of these 12 patients, the 'native' saliva had been negative for
HCV-RNA. Patients with HCV-RNA-positive toothbrush rinsing water showed
no significant differences from those with negative rinsing water with
respect to certain clinical, biochemical and virological parameters. In
conclusion, our study demonstrates a contamination with HCV-RNA of a
considerable portion of toothbrushes used by hepatitis C patients,
suggesting at least a theoretical risk of infection by sharing these
objects and strengthening the recommendations to take care of a clear
separation of these personal care objects between patients and their
household members.

An anecdotal reflection: Prior to diagnosis, I shared an electronic
toothbrush with a family member. Rationale: The electronic brush
provides superior dentail care (and we only had one). After I was
diagnosed with HCV, we immediately stopped sharing the toothbrush.
Rationale: The toothbrush presented an indirect but blood-to-blood
means of transmisssion for HCV. My family member subsequently tested
negative for HCV, but breaking the taboo against sharing a toothbrush
now carries an entirely different implication. Reason: A toothbrush
shared presents a means of transmission for HCV. And about that razor I
borrowed from an HCV-infected acquaintance...    -bg

I'm worried that someone with the virus will sneak into the house and
use my toothbrush.  I plan to use the toilet bowl brush for my
teeth.....I don't think I need to be concerned about someone breaking in
and using the toilet bowl brush.  Probably go thru toothpaste faster,
tho.

Get real, nobody is going to use elmo's toilet after he's cleaned it
with his toothbrush. -bg

Is it a typical symptom of post-tx recovery for patients to associate
the utility of a toilet bowl brush with the dental hygiene requirements
of elmo? I am quite disconcerted as I am now in the first weeks of
post-tx and am recurrently haunted by mental images of poor elmo stuck
with the business end of a toilet bowl brush in his mouth. I understand
that diligent tooth brushing might actually provide for optimal dental
health, but the sight of a huge brush in elmo's mouth has me alarmed --
I'm afraid that these visions I now suffer are simply not normal and
may be a harbinger of even more frightening hallucinations to terrorize
me as my recovery continues. What might I expect in the coming weeks
and years? Could this be what my life will be like indefinitely? -bg
