ALT and AST during tx

I've just had my labs back after 9 weeks of tx.  Everything was looking
pretty good. I had a 3 log drop in VL at week 4 and ALT and AST have been
down in the normal range. Neutrophils  have been taking it a bit hard
requiring interferon dose reductions on a couple of occasions. My latest
results show neutrophils are back up to 1.4 g/l (which is good) but ALT and
AST have pretty well doubled, taking them out of the normal range Alt is 64
(normal 5-50) and Ast is 53 (normal 10-45).

The first time I did tx I recall that Alt and Ast were quickly normalised
and remained that way the whole time. That was non-peg interferon. Does
anyone know if it is normal (ie not a bad sign for eventual SVR) for Alt and
Ast to be highish during tx?

My doctor has mentioned the possibility of Neupogen for low neutrophils
rather than an interferon dose reduction.  Anyone know at what point is
Neupogen usually recommended rather than simple dose reduction?

Experiences vary, but elevated ALT/AST isn't "abnormal" under TX. While my
levels rapidly normalized in the first few weeks of therapy, others here
cleared the virus and ended therapy with ALT 3x times the normal high limit,
but eventually normalized. And they obtained SVR.

So I suspect enzyme levels are all over the map and cannot be counted upon to
hint of the success of treatment.

As for the Neupogen to battle crashed neuts: I don't know what the recommended
level is to trigger intervention, but if offered, you should surely take it,
as opposed to tx-meds dosage reduction...

My neuts (abs) have dipped very low (.73 most recently) but my doc has
not been too worried about them. Said that he'd never had a problem with
an otherwise-normal hep-c (now that sounds odd) patient. Wasn't
interested in prescribing growth-factor for the WBC's (but had no
problem prescribing Procrit for the RBC's).

I'd fight very hard before I'd let my doc reduce my dosage.

I've heard that AST/ALT are just markers and that you can't really use
them for much else.

Thanks for your comments. Good advice as usual. Basically I shouldn't worry
too much about ALT and AST and I should do what I can to keep up the full
dose of interferon. Just thought there may have been a rule (like if
neutrophils fall below 750 for 2 weeks in a row then start using Neupogen).
But I guess it's up to each doctor.
