White blood cells

Just got a call from my doctor's office.  My white blood cells count is 400
but should be at 750.  They are going to add another injection to take along
with my Peg.  Also they want me to reduce the Peg from 0.5 to 0.3.  Does
this sound familiar to anyone?  I'm not feeling bad after my second shot,
but I could really use some good news.

This is my opinion based off  my experience and it is,  "it is way to early
to cut back on the Peg". Your chances of SVR are not good taking the full
dose (assuming your type 1). By cutting back your Peg dose so soon , your
odds get that much worse!

Right now I am taking neopogin twice a week and for a long time 3 times a
week to maintain my WBC count at 1.5 to 2.0 (150-200).

If you take the neupo and your counts maintain, there is no need to cut
back. Every article you will ever read will say your counts are going to
drop because of treatment.

The hard part is telling your doctor he needs more experience. They don'
like us talking treatment behind their backs!   Just my opinion.  Buster

Yes I am type 1.  I didn't think reducing the Peg was a good idea either,
just my feeling. I do understand what you are saying.  However, this doesn't
look like a situation that could go on too long.  I may start looking for
someone else soon, or do you think that would be jumping the gun a bit too
soon also?

I'd say you need to have a frank discussion with your current doctor, to make
sure he understands what your goals are, and allow him to explain what is
driving his decision making process in your case. I think you have to allow
him that much before cutting him loose.

To be better prepared in advance, you might read the Product Information sheet
that comes with your Peg-IFN shots, which will point to Absolute Neutrophil
Counts ("ANC") as the primary controlling factor, not overall WBC counts.

I guess the thing to do is tell ask him if he would hold off on the peg
reduction and monitor the Neupogen. If he says no, you can start
interviewing another doctor and see how they feel. You can start the
treatment over like you never started if you find someone else. buster

AAHHHHHHH!!!!!!  Dose reduction?  Your doc is setting you up to fail tx.
He should give you a chance to respond to the Neupogen before he
considers dose reduction.  After all, you feel relatively good, don't
you?  Your absolute neutraphil count is more significant than WBC's.
How are they?

Yes I do feel good.  The symptoms after the second Peg shot are nothing
compared to the first.  I may take busters advice.  Yes it is Neupogen, once
a week.  I don't know what my neutriaphil count is.

Roaid, my routine is to get the lab work on Mondays and watch the counts
closely.  At one point the wbc went down as low as .5 (50) .8 and .9 but we
did no reduction. Sometimes I wonder if we should have though. What the doc
was doing was tuning the Neupogen  dose in. Twice a week now and I have done
48 weeks then failed and am now on 2nd round of treatment  week 55 of 72.
With no dose reductions. Here again all the twicking was done on the
Neupogen dose and not on the peg dose. Same thing happened on the red cells
also. buster

I haven't read all of the posts yet so you have probably already heard
what I'm going to say. My first time through tx my blood work was so bad
they had to reduce my dosage so I could continue. By my second round of
tx Neupogen and Procrit were available and they helped keep my counts up
so I could continue tx at full dosage. If possible try to maintain a
full dosage of tx and check on Neupogen, it may do the trick for you as
well. In my case tx didn't work, but it wasn't attributable to the
reduction of dosage. The more factors you can keep in your favor the
better the chance you will have at succeeding.

idea what I should do now, or at least what I want to do.  Printing some
