Glaucoma, Cataract Surgery and Light Flashes

I had cataract surgery a year ago and noticed from the beginning there was a
glare and/or flashes of light from my lower left eye after the surgery.
There is also a "pulling" sensation.  Both the flashes and the pulling
aren't as noticeable when wearing glasses, so even though I now have 20/25
vision in my left eye after the cataract surgery, I still wear glasses.  I
have distance, reading, sun and computer glasses!  I was told this was from
the vitrous gel pulling away from the back of the eye.  Eye pressure is now
20, down from 22 or more.  Also, the left eye continuously itches.  I've
always had itchy eyes, but now it's worse.  I've tried every eye drop on the
market.  None of them helped.  I guess I should be thankful that Timolol
seems to work for me in lowering IOP.

Is there an eyedrop I haven't used that would control some of the itching?
I'm resigned to wearing glasses almost constantly because of the
flashes--though not happy about it.  Someone told me the "pulling" was
because I have a cataract in my right eye also.  Yes, I do, but not enough
to warrant surgery at this point.  I don't know if I could tolerate two
flashing and itchy eyes!  Should I get a second opinion?  I, too, live in
northern CA and am a Kaiser patient.  I read Dr. Cohn's answer to Pete about
the glare.  Since I was told it was from vitrous gel pulling away from the
back of the eye, should I just learn to live with it?

Hi Pat

I read your message with interest particularly as my left ( cataract
operation ) eye also itches.  I have worn spectacles for 60 years since the
blitz dropped a house on me and I was buried for three days.  I am resigned
to specs I just do not want to lose sight in my right eye which is affected
by glaucoma and a growing cataract.

You spoke of Dr Cohn's reply to ' Pete' (?me?).  It hasn't appeared on my
screen.  Have I missed it?  Could you copy it to me direct?

Itching can be caused by allergy, dryness, or more commonly,
blepharitis, inflammation of the eyelid margins.  I can't tell you if
there is an eyedrop you haven't used that would control your
itching...you neglected to mention the name of any of the drops you
have used before.  There are a host of drops for allergies that work
well: Patanol, Optivar, Elestat, Zaditor, and Alamast are the major
ones in use.  For blepharitis, I prefer a mild steroid like Alrex or
Lotemax if artificial tears alone don't help.  I also have these
patients use hot compresses twice a day and scrub their lids with baby
shampoo at least once a day.  Having not seen your lids, I don't know
if they are inflamed or not.
I remind patients that steroids like the last two I mentioned may raise
eye pressure in a small segment of the population, and therefore I
avoid long-term treatment with them, and I follow the patient's IOP.

"Pulling" sensation is usually due to a difference in the refractive
error between the two eyes (differing eyeglass prescription between
right and left eye)...not uncommon if only one cataract has been
removed.  It can also come from having an incorrect prescription in
your current glasses, so have your doc recheck your refraction.  As for
flashing lights, yes, that comes from traction of the vitreous gel on
the retina...nothing you can do about this.  Eventually it will stop
when the traction bands break.  This is not caused by cataract surgery
but can become more prominent afterwards.  Hope that helps.

First, thank you, Dr. Cohn.  Where have I been all my life that I haven't
heard of many of the eyedrops you mention?  I've used Murine, Visine (both
plain and allergy type), Tears II, Bausch & Lomb moisturizing and probably
others in the past which haven't helped.  I've never been told I had
blepharitis, though one optometrist told me about baby shampoo which I use
on eyelids.  Along this same line, I avoid like the plague getting face
cream near my eyelashes.  And eye makeup?  Forget it!  Interesting about the
hot compresses.  I'll try that.  I've been using cold water.

Thank you for the suggestions.  I've been plagued with itchy eyes all my
life, and I'm now in my 70s.  My sisters also have the same problem, as did
my mother.  Must be hereditary!
