I had trabs in both eyes about 12 years ago. Today after all that time, it
appears that one is plugging up. Over the last 2 months, the IOP has gone
from a stable 10-13 to 20 all in 60 days. Since it was first noticed about
60 days ago at 15, I was put on Timoptic for a month, no results and it maintained
15, then Alph for a few weeks to 18?? and then Lumigan for about 10 days
and it climbed to 20. In all your combined vast experiance, other than back
to the scalpel, are there any other options? If it's the scalpel, how urgent
is it in days before I should head to the operating room? Before I noticed
the sneek thief the first time, I had already lost 80% in my right eye and
40% in my left eye which is the problem eye now. Help!  Thanks..

> To be honest, Tom, it would appear that a repeat trabeculectomy, or some other type of filtration surgery, would be your best bet, especially if you already have significant optic nerve damage (and therefore should have an IOP between 10 and 13 on a regular basis).  Good luck to you.

DR C --My IOP is 18 1/2 in the left eye and 16 in the right eye --is
this something to be concerned about ? My Dr. said the optic nerve is in
good shape -but because of other smaller numbers I read here -I wonder
???

This all depends on the health of your optic nerve.  If your nerve
looks perfect, then your IOP is just fine.  If your nerve has mild
damage with no field loss, it still may be okay.  If your nerve has
moderate or severe damage, that IOP may be too high.   You've gotta
take this up with your own doc...ask him/her where your IOP should be
for your nerve.

Had glaucoma diagnosed 30 years ago and again 8 years ago and as high as 30.
Still have 20/20 vision, good peripheral vision. Have not used drops for may
years. Pressure is not necessarily an indication of Glaucoma. Suggest you
get several glaucoma specialist professional opinions before you agree to
surgery. al.

Hmm, that's interesting. I thought professionals would all recommend at
least eye drops at this kind of IOP. On the other hand, I understand that
cornea thicker than normal would give *false* high reading. Have you ever
had your cornea thickness measured?

Also, younger people could withstand higher IOP. At what age was your IOP of
30 measured? What was the trend of your IOP over the 30 years period?

Anyway, good perpheral vision may not mean that there is no optic nerve
damage. Hopefully this is not your case.

My reading on thickness of the cornia was 519 ? good or bad ?

average is about 540.  For every 20 microns less, add about 1 point to
find your actual IOP...for every 20 microns more, subtract a point.
For you, a pressure of 20 is probably really 21 because your cornea is
about 20 microns thinner than normal.  Thinner corneas are easier to
flatten at the slit lamp with the applanator, so the pressure you
measure is artificially lower than it probably is inside the eye.

Flatten at the slit lamp !!--ee gods -that's scary !!!--lord I have SO
much to learn , I am such a "peon " in this group ; ) -tho I Have
learned just enough here that my doc does give me a little more respect
; )
so my IOP is 20 in one eye and 17 in the other --I can ad one point
-higher math --not my forte .Yes --we are lucky to have DR C here with
us !!!

Just wanted to say thanks for all the comments. I'm off to CA to get something
done about pressures next week. My Doctor talks about "needle surgery" to
see if he can get the trab to work again. Local doctor said that she can't
figure out why it's not working as it looks perfect. We'll see (I hope)!
