A friend of mine recently had a grand mal seizure (flopping around,
grinding teeth, ect followed by 1-2 minutes of unresponsive
'paralysis'). The seizure was caused by an all-night cocaine binge, and
he has never had a seizure before. Immediately following the seizure he
was very disoriented (couldn't say what month/year it was) but by the
next day seemed mentally sharp. However now he has extreme
dizziness/vertigo whenever he stands up -- to the point where the room
feels like it is spinning. He takes about 1-2mg xanax daily but never
had this side-effect (and obviously isn't on anti-seizure meds YET). He
is a pretty healthy 30 year old otherwise, and the vertigo has been
going on for about 3 days now.
I read that a grand mal seizure can cause oxygen deprivation -- is
there a chance that this would cause permanent brain damage? Or is
vertigo a common 'hangover' of a grand mal seizure? He has an
appointment to see his GP, but I wanted to get some other input from
you all. Any advice/insight would be greatly appreciated (and yes he's
quitting hard drugs, this was the lesson he needed -- maybe somebody
using will benefit from seeing this thread).

One should go to the ER for new onset of seizure.
Here's what I'd be worrying about: A cocaine binge can raise blood
pressure high enough to cause a stroke in a healthy 30-year-old.
Strokes sometimes precipitate seizures, and vertigo can be a symptom of
stroke.

Did your friend go to a doctor after this seizure? The all-night cocaine
binge sounds rather damaging to me. To answer your question, I don't have
vertigo after a tonic clonic seizure (grand mal).
