Update - MDI and me

Why I am posting this I dont know other than to update those who actually
care!
If u are waiting to slag me off again dont bother posting!

Ok so today I called the DSN team to report in as asked to call in every few
days by the endo consultant.

But since starting MDI on the 14th and doing my increase on the 16th we have
a little bit of a change.
brekkie readings still between 7 and 8mmol
lunch readings still between 14 and 16mmol
teatime readings between 16 and 18 mmol
Night readings are down to around 19mmol from 21mmol

And the DSN has advised me too:
brekkie novorapid up by 2u
lunch and dinner novorapid up by the 4u the endo recommended to up by

So regime is now:
Brekkie
 a.. Lantus 46u
 b.. Novorapid 4u
 c.. Metformin s/r 1000mg

Lunch
 a.. Novorapid 18u

Dinner
 a.. Novorapid 20u

I know the night reading is only down by about 2mmol but its a change for
the better and I have been waiting months for any change!

So I am all bouncy and happy today!
But am still a little frustrated that its not alot better (but its still
improvement hey)!

It is an improvement, if only a small step towards what you want.

I'm not going to offer any suggestions as to what you should be doing as
that would be wrong. However I'll just relate what I have seen in the
past and also experienced. It seems like, when you attack the T2
resistance problem, there is almost a situation like a 'critical mass'
before all the factors start to line up.

Treat the resistance with metformin, and then add insulin, then you
reach a point where the changes appear to accelerate. That 'critical
mass' is what you are heading towards at the moment. Once all the
factors line up, it'll swing downwards in a pronounced fashion.

It's almost like having to climb a hill to get to the plateau where the
road ahead starts to become visible, you just need to get to the stage
where you have climbed that hill by finding what the correct diet,
insulin and metformin combination is to overcome the hill of your
insulin resistance.

Keep on climbing that hill, but remember at the other side it can be a
steep road back down. Testing is even more important then as you need to
identify the right balance and avoid hypos.

 I'm guessing that your health problems mean that,  like me,  you will be
controlling your blood sugar with insulin for the rest of your life.    Or
at least,  in your case,   for years to come.

That suggests that the time may come in which you control using the same
technique that I use  (Called "DAFNE" in the UK),  i.e.

How much insulin do I use?    As much as I need that day.
How often do I inject?    As often as I need to that day.
And when I wake up in the morning,  I have no idea of what either of those
numbers will be that day.

But I keep records and tell the doctor once every six months. . .unless I
tell him once every year.

I suggest that you start to think about how you might handle my method  in
days to come.

We had a poster  (Opinion8) who started out using your method.    She said
that she was afraid of my method because it was too hard.   I told her that
it would be easy after the first 1000 injections.

She wrote me later and said it got easy after the first 500 injections.

woohoo, it's going in the right direction, hopefully you can
experiment successfully to avoid the mixed insulin ;)

I do care, but I have absolutely no knowledge as to the medical side of
your problems. I can't be much help there, but any improvement is better
than no improvement, so good for you.

I been on metformin since september I think - started on normal release then
went to slow-rlease.

Been to gp today for chat and get one or two things checked on.
One item I was worried about was burning pain at times in left foot and pack
pain and leg pain - But I have a mechancial back injury it seems - back
muscles were in spasm when gp went near me so she given me some muscle
relaxents for when it gets very bad.
But at least it wasnt neuorpathy which I was worried it was.

I have an issue with severe dry skin since diagnosis and insulin but been
told its all related and given some cream - similar ingredients to one I
been tryign from a sample but is cheaper for gp to prescribe.

Going into my gp in new year for full check-up review and blood tests (only
gp can do mine).

But she said things are doing better now - readings have stabilised and are
starting to turn down a tiny amount but as one of you said - gotta get to
that level where things just turn around and drop then we will work out a
slightly different regime to keep them down.

Shes very supportive of me being in control as the consultant has said too.

But it is definite that I will "NOT" be going onto mixed insulin ( I dont
eat regularly enough or something) and basal-bolus is my partner now lol!

Thanks folks - its not been an easy route but least we moving now!

type 2s on insulin should expect a slower rate of improvement compared
to a type 1.  just keep working at it.  You and the doc will get the
correct insulin levels soon enough.

I didnt realise it would take longer - I hues thats why they keeping
explaining it needs to go slower than I want but not as slow as DSN wants
either lol
Cinsultant here can up by larger amount than a DSNlol

I got fingers crossed as night time is creeping down slowly lol
