Logging of Insulin Useage

I'm curious how you insulin users log your insulin useage.  

Is it by Basil or Basil and the units used?

Is it by say PreBreakfast, PreLunch, PreDinner, etc for short term
insulage useage?

I'm using a pump and a One Touch UltraSmart meter.  Even though my meter
holds all kinds of information I also continue to use a paper log book that
I get from Minimed.  It's the *only* paper log book I've ever used that I
actually like.  In the log I mark off my basal rates in the proper spot.
Don't always mark them in every day, sometimes I skip days unless I make a
change.  When I bolus for food or for a correction I mark that in the proper
box.  I enter all bolus info in my meter as well.  The UltraSmart doesn't
keep track of basal insulin.  I know that if you're on injections you can
set up more than one type of insulin, and that will allow you to track both
kinds.

My pump also keeps track of insulin usage by basal & bolus and will break up
the bolus amounts into those for food & for corrections.  I have no idea if
I can get that info out of the pump and into my computer because I don't use
any of the Minimed software, and you also need to set up the BD Link meter
to download anything.  Too much trouble for me.  I do check my total insulin
use right on the pump.  It holds the last 31 days and I can average it over
any number of days, and that's where I can see how much was basal and how
much was bolus.

When I have an appointment with my CDE I bring the logbooks used since my
last visit and I also do some printouts from the Lifescan software.  She
looks at the printouts and if she wants to check on specific readings she
looks in the logbooks.  The Lifescan software has a bunch of different
reports to choose from.  I can also check insulin usage on the meter and I'm
pretty sure it can be sorted by meal, but I don't really look at the meter
info for that.

strip and insulin makers) allows you to record all
of the above
