I had a hamburger for lunch today (minus the bun) and some diet pepsi so
there was almost no carbs and so a little later my level was 49.. I drank
some OJ and had a little piece of cake and it went up to 116. about 2 hours
later it was at 77, I felt fine so I didnt do anything about it. and now I
justed tested and its at 145/138 (took it twice to be sure), what the heck
made it go so high again?

Probably a liver dump. Next time try a light snack if you are in the 70's.

First, I would need to know what (if any) meds you are on. Those things can
affect your BG. And is there a reason you ate almost no carbs for lunch?
If I did that, it would be a sure route to hypo unless I were cutting back
on the carbs to offset a high BG prior to eating.
And there is another unknown. You say you tested at 77, but you don't say
how much time elapsed between those readings. When I test at 77, I need to
eat. If I don't, I'll go hypo, usually rather quickly and sometimes without
symptoms. My liver will then dump glucose to compensate for it and I might
have high BG.
It might be a good idea for you to include some healthy carbs in your meals.
That way your BG will be more stable and you won't be having these wild
swings, then having to eat or drink junk to compensate.

Sounds like you were near hypo and your liver freaked out and dumped some
glucose. That's my best guess.

I would eat a hamburger on a bun and then go from there. You may be able to
have the bun. If not eat a slice of bread with the burger. You need to have
some carbs in there but not too many. I had the same problem. You just have
to figure out how much carbs to have without spikeing.

A couple of things may have happened...before or as you were eating your
cake you may have had a liver dump. Also, just because there are no carbs
in your burger doesn't mean it won't raise your BG. Most T2's still produce
enough insulin to handle moderate glucose amounts...the protein and fat in
a burger will still convert to glucose, just at a much slower rate than the
carbs in a bun. You felt low...had some OJ and cake. While the insulin you
produce was handling that load of glucose the fat and protein from the
burger were also breaking down. It's possible your body ran out of insulin
and wasn't able to handle the full job.
