My Dr put me on the Metformin XR yesterday. The prescription states 2 metformin tabs per day. Does that mean I take both tablets at once or twice a day like I did the regular metformin? Jacquie, when I was put on the Metformin, I was told to take one in the morning with breakfast and then one at supper time. They should have said horse pills:) By all means, call up the doctor or the pharmacist and ask them what the dosing schedule should be. HOWEVER, metformin can cause intense gastric upset in some people, so many people find it best to start out with one Metformin, take it for a few days until the side effects calm down and only then add the second one. Taking the two tabs together the first time might be more than your body can handle. I felt poisoned the first week I took Metformin and it was very worrying. But it passed, and I started feeling fine and losing weight without dieting while my post-meal blood sugars dropped to much more reasonable levels. My understanding is that the timing of this med is adjusted to avoid GI side-effects and doesn't affect the efficacy of the drug. I've noticed no difference in my response between the times when I took the doses together and the times when I split them up. Your clinician is the one to check with for the definitive instructions for how you should take the med. Priscilla I looked at my prescription bottle and mine is Co-Metformin. I don't know wht your doctor wants, but I take 1000mgs in the AM and 1000mgs in the PM, breakfast an dinner, and all XR. It works for me, keeping me free of the trots most of the time unless i eat some sugar alcohols! Wendy Thanks Guys, I called the Dr's office today and they said take two at once. For extended release metformin the prescribing info says to take at one time. Despite claims of it reaching a steady state, there seems to be some peaking. Bernstein says on P. 229 of "Dr. Bernstein's Diabetes Solution" that the timed release Metformin seems to have a peak 7 hours after you take it. I have noticed slightly lower morning blood sugars if I split my dose and take a dose at night. Unfortunately, I have also noticed that it seems to make me pee more, which makes me wake up more, which rules out a night dose as a useful strategy. The other thing about Metformin ER is that different companies' versions may vary in potency. When my pharmacy changed manufacturers and gave me a different shaped pill of the same dosage, I developed gastric symptoms for a few days just like what I get if I boost my dosage. Other people have reported similar things. What I have found over the years about Prescribing Information is that the data is based on statistical findings drawn from group studies done with people with very high blood sugars. Recently I changed from metformin ER (PAR Co.) to regular metformin with no change in dose. I thought I was seeing a real change in FBG down to 110 range. This lasted only a week and then back up to the 140+ range again. It was a real but transitory effect. It seems the body kicks in compensitory mechanisms to keep the status quo. One reason I wanted to go to ER was because I had a hard time remembering to take that second pill....plus the regular Metformin made me really nauseous...the other gastro stuff went away after the first few months of taking it. I am so far having no gastro problems with the ER(knock on Wood) I am in my 50's so get up in the middle of the night to pee anyways, and have seen no change. I have the same problem with not remembering them if they get split up. The getting up at night problem is worse for me than most people as I have a damaged bladder which can react to medications pretty strongly. Most people probably wouldn't experience that difference. As it is, I already wake up 2 times every night. The third time is killer. . . Sorry about your bladder problem...I had to have bladder repair in my 30's it wasn't fun. I like taking the Metformin at one time...and so far am not having any side effects at all...now if they could just make them a little bit smaller...LOL..:) Signaturejacquie I don't suffer from insanity....
