They run their course. This is almost a size that we'd call a "major"
aphthous ulcer. I've heard of these being treated with steroid
ointments, but you don't want to overdo this.
In the old days we'd use phenolics, or trichloroacetic acid to burn the
surface and cover it with a slough of dead tissue. It felt better
immediately, but it then takes longer to heal. Most conservatively,
they make OTC preparations such as Orabase or Zilactin which cover and
protect the ulcer to minimize irritation on the teeth.

Well, so do the escharotics--by denaturing the proteins (IOW, chemical
burn). The laser may do it in a more controlled way, so that the
healing is faster--which is my objection to the escharotics--that
healing is generally retarded. I believe this is one of the claims of
the laser manufacturers--that it does not retard healing. I think I've
even heard them claim that it accelerates healing--but I don't see how
that can be. Certainly the patient feels better either way.

My experience has been immediate relief in pain but healing of the visual
lesion still takes the same amount of time. I once had a patient burn the
palate with hot soup (scooped up the oil floating on the top while
tasting--during cooking). Burnt about a third of the hard palate. I
treated with the diode laser every three days for about ten days on that
one. Aphthous ulcers only need one treatment. Takes all of 5 minutes.

I had to google this. Sounds like polyrazzmatazz, but when I looked it
up, it sounds like one of the old escharotics. According to this page,
it is a discontinued medicament that they can custom compound for you:

Polyphenolic sulfonic acid.
Cauterized nerve endings in about 5 seconds.
One treatment, no apparent delay in healing.
Afterall an untreated apthous ulcer will heal in 7 days.
A treated one will heal in one week.
It's old school, tried but true.
Patients appreciate the relief, though application burns a bit.

I put a big pinch of alum powder on it, let that sit for 15-30 seconds,
rinsed it off, and then slathered it with Orajel.
Alum to shrink it. Orajel to numb it. Seems to work, albeit quite
modestly and very temporarily.
