I have a right top molar that has had a root canal and is scheduled for a
crown in Dec. Tonight my gum was sore, like I had a popcorn kernal stuck in
it, so I gently flossed and a very small chunk of tooth came out from between
the teeth. I can feel a small hole but due to it being so far back, I cannot
see where exactly it broke. My question is this: Will I have to have it
pulled? Or can I still get the crown and save the tooth? My gum is kind of
sore too but Im assuming its b/c of the piece of tooth being lodged in there.
The tooth itself doesnt hurt (its dead) but the gum is sore.

It's difficult to tell, but if it was a small piece it might not be a
big problem.  Are you sure it wasn't part of the temporary filling
that came out?  If your gum is sore, it's possible that a cusp has
broken off deep at the gumline.  You might need crown lengthening to
get access to the broken area of the tooth.

These are of course just possibilities so it's better to have it
examined by your dentist to get a definite idea.

The piece was pretty small, I needed a tweezer to pick it up, and it was
whiteish. The filling in the tooth is silver and I was told it was a
permanent filling. The gum doesnt hurt at all anymore so maybe it was
irritation from having the piece of tooth stuck there? Im calling the dentist
today regardless b/c its very important to me to save the tooth and not have
it pulled.

sounds like it was a small piece of enamel or a portion of a fractured
cusp. Most likely this piece would have been removed anyway. I'd get
it checked but not worry about it. Your dentist has chosen to use
amalgam as a core build-up material. I do not use this material since
i believe there are better build up materials out there ones that bond
to the walls of the tooth and do not cause the tooth and gums to stain
black due to the corrosion of the silver. But it should be fine if all
you want to do is prevent the loss of your tooth. Hopefully you had
this discussion with your dentist, if not it is still considered by a
portion of the profession to be an acceptable material and philosophy
of practice.
