Exposure & Bracketing of an Impacted tooth

Hello everybody.  I have an Impacted Canine that needs to be exposed
and bracketed.  My Orthodontist has already given me a referral to
have this performed by an Oral Surgeon.  However, she was unable to
provide me with any type of price estimate at all.  Also, I would need
my primary tooth extracted to make room for the new one.  I have
called around to almost every Oral Surgeon in the phone book and have
had no luck in recieving any type of quote or estimate.  They all say
that I need to come in and have the Oral Surgeon evaluate my case
before they can help me with an estimate.  My insurance does not cover
any Oral Surgeons in the area so all expenses would be out of pocket.
I don't have the money to go around to each and every Surgeon just to
see what a price might be.  Does anyone happen to know the average
pricing for such a surgery?  Any input would be greatly appreciated.

D7421 - extraction, simple - any tooth

D7280   - suricgal expousure of an impacted tooth.

D7283    - placement otrho bracket at time of surgery
