apthous ulcers, canker sores

I recently had a 13 year old new patient present with significant
crowding and multiple canker sores on the inside of the lips and
cheeks.  interestingly, these canker sores are more or less at the
level of the occusion whether that has anything to do with anything.
his mother had him to the pediatrician and was given the standard
answer of maalox/benadryl or lidocaine as a paliative approach.

his mother feels this has effected his sleeping and his eating and he
quite frankly is short for 13.  he is also a mouthbreather.

I'm wondering if any of you wize kids know of any preventive approach
or systemic drug that can help this patient.

First step would be finding a toothpaste without sodium lauryl
sulfate.  Some reports have that reducing frequency by 2/3.

Has the physician done any checks of his immune system or allergies?

Thank you kind sir.  Rembrandt used to be on the list, but the
last time I perused the toothpaste aisle in Wal-Mart (main shopping
venue of my clientele), I could find no Rembrandt versions without
SLS.  Same for every other tube from every other brand that I picked
up.

I'll take this new list out with me and recheck things.

For Newbie, the explanation had something to do with SLS being a type
of detergent.  This detergent can help break down the mucosal lining
on certain individuals leading to more lesions.  It's seemed to help
a few patients over the years, but I don't get a lot of cases where
it's a problem.

    Apparently it can be, though I don't recall the context in which
non-SLS toothpastes were being discussed.  I just saved the list on my
desktop for handy reference.

Common apthous ulcers are cause by a sensitivity to "transitional *L*-form bacteria".

Giant Recurrent Apthae are a different animal, as they cause scarring.

Generally applying Negatan, i.e. Negatol 45% compound; (Sulfonic Acid ?) to the ulcer will palliate
the pain. Paint it on with a swab, leave for about 15 - 30 seconds, then clean the ulcer with wet
gauze.

For the rinse, a cocktail of Kaopectate (Kaolin)/Tetracycline/Benadryl
with or w/o viscous lidocaine can be used. (equal parts)
Swish and swallow.

Suppose that in a severe case you could try the low dose tetracycline
that is used for acne, or the "PerioStat" thingy.

For a one time outbreak you could treat the surface tissue with a laser, but
you may not be able to do that every three weeks if the lesions are
recurrent.
