Root Canal Question

#18 tooth

I had the shallow mercury filling replaced with composite, 3 surfaces.
The only area that hurt right before the procedure was the rear-cheek
side crown area, when he tested with bite stick. I had been having
some minor pain (2/10) with chewing. Xrays showed that the root area
of that side was fuzzy; you could not see the "line" extending all the
way to the bottom. He felt the root may be problematic, but said he
wanted to try the composite replacement and to see if a crack was
there. Dentist took most of that crown off, said he found no crack.

9 days after this procedure, I get minor pain (2/10) when I clench
down in that same area. It does feel abnormal when clenching. When I
floss between #18 and the rear molar, the pain is about a 5/10. Very
slight cold sensitivity, none to hot. Pain with chewing seems less
than before filling replacement. Now less than a 1.

Dentist called me and when I told him the update, told me to see an
endodontist for root canal evaluation. I have the appt this Monday.

Any thoughts?

You are doing the right thing.  Let the endodontist check the tooth--it
is not uncommon for teeth to have ambiguous symptoms.  A good
endodontist will not recommend root canal unless there is a high
probability that it is necessary.

I posted the below about a month ago.

I saw an endodontist. He applied heat, cold and had me bite down on
the affected tooth with a q-tip on top of it. I had no pain at all at
any time during the exam. Another xray did not show anything
significant. He said he would not do a root canal now, but it was
likely that I would need one in the future.

Now a month later, symptoms as stated below still continue. I find I
am unconsiously avoiding chewing on the affected side. Flossing still
hurts. The tooth just does not feel right, but the pain is not at all
untolerable. Cold seems to hurt slightly at times.

Should I just wait for this to get worse, or take care of it.

IMO take care of it sooner rather than later.
Waiting can only make treatment more difficult.
The pulp <nerve> chamber and canals decrease
in size in a chronically inflammed pulp.

Sounds like you may have a cracked tooth and
are probably a clencher/grinder.

But that's just a guess from this computer monitor.

One could try replacing the restoration in case it did not bond well enough
interproximally.  Personally, I would like to see an NTI in place for 2-3
weeks.
