Should I file a complaint on this dentist?

My daughter had a small cavity on both side of lower back teeth. I took
her to a dental office. There were 2 dentist in the office. The first
dentist clean her cavities and put composite resin fillings on both
teeth. But just in few months ( 2 or 3 ), we found cracks between the
fillings and teeth.

And then we went back to the office, and the other dentist said this
was because new cavities. He said he needed to do Pulpectomy instead of
Pulpotomy order to avoid teeth inflammation on one side, the other side
just refilled. But this time is even worse. In a month, my daughter's
face was swollen on the side which Pulpectomy was done. We went back,
this time the dentist said he cannot
do nothing except taking out the tooth or he can refer us to a
specialist. One month later, the tooth on the other side which just was
refilled was swollen too.

Can anyone give me some ideas if this is very common  in the dental
operation? Just in less that one year, a small cavity turned to such a
worse case? Should we file a complaint against this office?
We tried to get all documents but the office just refused to give us
any copy.
