Lone standing tooth pain

Hi,
i have been having trouble with a tooth (i think) for 3 months.
the reason for my doubt is that the dentist having looked at an xray
says there is no infection, (there is an old filling there ). she says
that the pain is due to its being a lone standing tooth.
it is the extreme right rear tooth(molar?).

The dentist has given me 3 options, extract, refill, or get a denture
made to support the tooth.

i went to the doctor during this time and he treated me for trigeminal
neuralgia (incorrect diagnosis)

The pain is all of the following, though not all at the same time:
1, throbbing pain in the tooth mentioned.
2, continuous pain in same tooth.
3, dull pain in front lower tooth (has a chip in it)
4, a combination of number 2 then moving to ear ache on the same side.
5, I have had "sensations" in the hand of the same side as the tooth
though not recently.

So for over a month i have been taking painkillers every 4-5 hours
(paracetamol and ibruprofen) , they work, but when they wear off the
pain returns.
I had it refilled, but to no avail, should i just get it taken out?

    No way to say without examining you.  There are a variety of problems
that may cause your symptoms that may not be readily apparent on x-ray.
    What was the reason your doctors felt you had a trigeminal neuralgia,
and how was this ruled out?
    Is this a lower or an upper molar tooth?
