Can anyone kindly tell me, in a nutshell the proper verbage and medical
reasons for having a bone graft on a bottom (last) back molar?  I pay
so much for dental insurance and they have refused me.  I am having the
tooth extracted today and want to do the bone graft as soon as
possible.

I guess they consider it "Cosmetic"...or something..they said that I
have an exclusion on bridge work, and dentures...dont know how that
falls in that category!!!  THANKS!!!

There are a variety of legitimate reasons a bone graft might be done.
If there is a specific exclusion you may be out of luck; OTOH the
surgeon KNOWS why he/she placed a graft and is best equipped to help
you.  Furthermore, they should be more than willing to do this, or
explain why it would not help.  It's no good your writing a letter of
medical necessity--the doctor has to make the judgement that is is
necessary.
