Did the hygenist damage my teeth?

Although I neglected my teeth somewhat as a youth, as an adult I've tried to
take very good care of them. At 47 I've had a moderate amount of dental work
done. Briefly, I recently went in for a cleaning and checkup.  After my
cleaning the hygenist conversation with my dentist clearly conveyed to me
that she pulled my 22 year old sons chart, who shares my name, rather than
mine. As my dentist proceeded to examine my teeth he stated for her to record
that several teeth should be watched for the first time in as long as I can
remember. Since this time I've had some moderate dental pain which is more
severe while brushing. My concern is that the hygenist didn't have the
correct chart to alert her to my existing dental work (composites, crowns,
caps ect.) so that she could avoid or use the correct instruments and
procedures to clean these areas. Is it likely that the hygenist damaged my
existing dental work because she didn't have the correct chart to examine
before and or during the process of cleaning of my teeth? Please reply. Thank
you!
