apicoectomy

Hi,

I had root canal done by a dentist about 2 months ago (the second time;
the first time was 3 months ago), but I continue to have sensitivity on
that tooth. Now, the gum around that tooth is swollen with infection.
My dentist referred me to see a endodontist who is recommending
apicoectomy. According to my dentist, something broke at the tip of the
root canal and got stuck in the curve line. According to the
endodontist, this piece is why i am getting infection. Now, he made the
diagnosis just based on what my dentist told him.
I have two questions:
1. Do I really need an apicoectomy? I had root canal done twice for
this tooth already, and I worry that apicoectomy is not going to solve
the problem
2. Can I ask my dentist for a refund or credit to go toward the
apicoectomy produce? Is this customary? The cost of apicoectomy is
around $1300.

Just add to the history of this tooth i chronological order

1. dentist  noticed a crack and recommended that I get an Onlay
2. after getting the onlay, the tooth becomes sensitive to temperature
and pressure
3. the detist recommends root canal
4. after the root canal, the tooth continued to be sensitive
5. dentist recommends to do the second root canal
6. The tooth continued to be sensitive to pressure (not temperature
though)
7. Endodontist is recommending apicoectomy

Good chance the crack has spread into the root and is providing a channel
for bacterial to get into the bone.  Teeth like this are usually removed.
No way to tell if this is your case or not.

Seem to recall an endodontist on this newsgroup, who knows of  helpful
advice for apicoectomy       However,  perhaps he has gone to Sri Lanka.

It's really impossible to say what's what without examining you.  That being
said, I would assume that the crack in the tooth extended further than it
appeared to the eye.  It could be a microscopic crack that is VERY hard to
diagnose.  Also, if a piece of a root canal instrument broke off and is
lodged inside the canal, then it would be almost impossible to seal the
tooth; that's why they are recommending the apico.  Will the apico
DEFINITELY solve your problem?  No one can say for certain, but often, the
procedure does.  The endodontist should be checking to see if there really
is a crack and base his treatment on that as well as other information he
will obtain.  As far as a refund, I don't see why the dentist should provide
a refund, assuming he did the treatment properly.  Dentistry -- and
medicine -- are not perfect sciences.  Everyone is not the same, and what
will work for you may not work for me.  There is not any way to guarantee a
treatment -- the dentist provides treatment at the standard of care and we
hope that things will work out; sometimes, they don't.  Good luck, and I
hope the apico will take care of your problem once and for all, but no one
can be sure ahead of time, and if someone says they are sure, I'd be
suspicious.

endodontist's office, the receptionist asked me if the dentist already
cleared the transaction with the insurance... This leads me to believe
that in case like this, the dentist may be responsible. After all,
isn't the dentist responsible if a piece of a root canal isntrument
broke off? My dentist is a nice person so I don't want to go in and
make unreasonable requests. But, if it is a reasonable request then I
would like to give it a try, because I am max out of insurance coverage
after all the work I did with my dentist. Thank you and I appreciate
your comments!

The endodontist office is simply trying to see what billing options are open
to them.  If your dentist has billed for the procedure (which would be
normal), than the endodontist will need to bill you for more of the
procedure cost, and possibly write more narrative to the insurance carrier.

Files do separate inside canals from time to time.  It is a fact of life.
Most times, they separate after the canal has been cleaned well, and sealing
around it works just fine.  Sometimes, a root will have strange anatomy with
extra curves and twists which do not show up on the x-ray image.  These
roots tend to grip the file tips and the ends can then separate despite the
dentist using proper technique.

Can I wait until January of next year then? Does it increase the risk
that I may need to extract my tooth?
