I agree Paul. In fact there are other errors too. Example,

Amalgam filling polished with pumice and hydrogen peroxide. Standard
post-operative amalgam filling instructions given to patient.

is also incorrect because amalgam should never be polished or
attempted to be polished until 24 hours after insertion. It brings
mercury to the surface if not completely set, and that alone raises
red flags.

I like your approach of using ( _ %) and (  _carpules) as it forces
whomever to be sure to EDIT the narrative to reflect the actual
treatment not the "canned response."

The original narratives were even worse! I had copied them from
someone who was not especially knowledgeable in dentistry but was
knowledgeable in Dentrix as part of a course I was teaching.

Here is the original. Of course there is no 1:20,000 epinephrine
concentrations. It would kill large game with one shot.

One suggestion in your chart note would be to say " N2O titrated to
clinically therapeutic percentage (%) for (  ) minutes, followed by
100% O2 for 10 minutes to end appointment.  The sequencing of events
is quite clear, as is the attempt to incrementally customize the dose
of N2O.

Its useful to automatically include narratives (progress notes) and
very simple to set-up. Dental insurance companies are getting very
fussy about what we are writing or failing to write.

So who uses narratives and who wings it? Anyone interested in sharing
a few narratives? Each one can be double clicked and edited at time of
treatment, say 3 carpules, not one carpule, and can be hidden so's not
to clog up the progress notes!

Sample narrative for amalgam

Sample Narrative for Composite Restorations

isolate the field, ligated tooth with floss.  Mylar strip used to
separate teeth.  Prep tooth.  28% phosphoric acid etch applied to
tooth for 1 minute.  Rinse for 30 seconds, primer and sealer to prep
area.  Light cured for 15 seconds.  Composite used shade A2.  Light
cured for 1 1/2 minutes.  Adjusted with football diamond.  Contact
smoothed with diamond contour strip, then checked for tightness with
floss.  Occlusion checked with carbon paper then smoothed with
finishing discs.  Resin filling polished with pumice.  Standard
post-Resin filling instructions given to patient.
