TEMPORARY CROWN

I'm wearing a multi-tooth temporary crown piece. Some of the filed
teeth are covered by a temporary teeth that extend completely beyond
the filed tooth to slightly below the gum line. I'm certainly aware
that a temporary piece is often ill-fitting - it's hardly a perfect
fit. But I'm concerned about the permanent piece. Shouldn't the
permanent piece fit perfectly over the filed tooth as opposed to
completing covering the entire tooth,all the way to the gum base? Would
appreciate responses from dentists - thank you.

    This is one of those areas where judgements differ.  Cosmetic need,
periodontal health, and caries protection have conflicting needs.
    Yes, the crown should end where the preparation of the tooth ends.  But
for front teeth, generally the preparation is carried SLIGHTLY under the
gumline so the edge of the crown doesn't show.  For back teeth, we
generally try to end the crown at the gumline.
    Sometimes teeth are short and the preparation must be carried far
enough down the root that the crown made will stay on.  In this case it
is wise to do what is called a crown lengthening procedure, in which gum
tissue (and sometimes a bit of bone) are removed, so the crown need not
be extended far under the gumline.  Extending the crown under the
gumline significantly will cause gum problems.

For an anterior bridge, I would imagine even you might
fabricate a temp once in a while.

The permanent bridge should have the margins cover all of the
tooth that was prepped.  The lab should be easily able to identify
where the prep ends and tooth begins and should match that in
the prosthesis.
