I am using carbamide gel (21%) to whiten my teeth.
At this time I use it for 5 nights every three - four months.
Could I use it once a week for a few hours and get the same results?
My goal is to have teeth that are always white instead of waiting a few
months until they dull.

This is overuse, IMO. This is a very concentrated gel. If it's been
sitting around a while though, it's extremely unlikely you still have
21% carbamyl peroxide, esp. if it isn't refrigerated.
Your teeth shouldn't be relapsing in color so fast. It's likely you
just have extrinsic stain which should be polished off, not bleached.

Be advised, that at this time, I think that might work, at this time.
May be even less frequently than that depending on diet / smoking / oral
hygiene.
