My fiance had his front tooth knocked out 20 years ago and had to have a
root canal and a fake tooth. The root is still there and has gotten
infected for the second time(they drilled from the top of the root and
filled it.) It has been okay for a month or so, but now it is infected
again. What are his options?

Many.

Sounds like he had an apicoectomy.
If that fails likely a vertical root fracture.

If root is vertically fractured...

extract --> implant -or- 3-unit bridge

The dentist wants drill on the root and try to fix it one more time.  Is
this a good ideal.
