Crowning front teeth??

Michelle - 07 Mar 2005 23:33 GMT I just had tooth #7 chip off, due to a cavity on the backside of it. My dentist restored it with a composite filling today, and told me that the only real tooth that was left there was just the shell of it, and that I would need to have it crowned sometime in the future. He said not to bite anything really hard with it, or the composite/tooth could chip off again. Well, I'd like to get the crown done so I wouldn't have to worry about that, but my dentist said that it's better to crown all four front teeth rather than just one. I'm assuming he means #7, 8, 9, and 10. Now there is nothing really wrong with 8,9 or 10. Should I be wary of this? Any info would be greatly appreciated. Thanks!

I'm assuming he isn't crown-happy, or he would have recommended you get them all crowned now. Old timers liked to crown all the front teeth because (except for porcelain jackets) most crowns just didn't look that natural, and doing just one would stick out like a sore thumb. With modern materials, it should not be necessary to crown multiple teeth just to make them match.

I have to get all six redone only five years later. Much chipping and breakage. And every time they redo it they have to chisel off more tooth and put stress on them, perhaps leading to another root canal. One of the teeth may have to have an implant, because there may not be enough tooth left to do another cap.
So if those other teeth are healthy, I'd leave them alone. Veneers are pretty but they come at a cost (besides money).

Either there was some problem with bonding technique, or you have some habits such as clenching or biting on stuff like pens, pencils, ice, or some decay problem. Porcelain laminate veneers have been around over 20 years now, and even though bonding materials have improved since then, many of the original veneers put in are still fine.

Veneer on #7 shattered and #8 partially avulsed (for the second time) lead to eventual loss of #8
Fights, nah, it was a bicycle accident first time. Second injury was due to a very slippery floor.

Emma, I forgot to tell you: I would also recommend getting TWO NTI's made; a day time one and a nighttime one. You may be surprised to learn how much of the wear is self-inflicted. Even if it's the first one your dentist does, as long as he understands what is necessary, it should go well. Cheers SP

If I was me (and sometimes I _am_), I would say protect your current investment FIRST: get an NTI and WEAR it at night (mandatroy) and in stressing times during the day. This will help you protect your dentition and buy you some time. Next, I'd tell your dentist that when the time comes to crown no. 7, you will go to the dental lab and get characterization stains done on your crown before it gets its final glaze. The technician does this like painting with oils on a canvas: a little blue here, a bit of brown there... It may not be perfect, but you can get the tooth to harmonize quite well this way. It would be harder if you had chipped nos. 8 and/or 9. You arrive with your teeth (I hope) and they put it in and paint on it this way.
A better way is to get the technician to come to your dentist's operatory and do his/her staining there, but this is usually more $$$.
If at the staining/characterization stage, they cannot get the tooth to harmonize with the rest of the anterior dentition, you must make the decision whether or not to get more extensive crowns done. Hope this helps SP
