Normal 3 root molar RCT over 2 visits access through a PV and then composite
filled. Tooth perfect for 2 weeks but now remains really heat sensitive
after 3 months, but not really to cold or pressure. After complaining,
edges/high points have been etched a month ago. Could there be persistant
residual infection ? How long would it take for this to manifest itself ,
if ever ? ie wouldn't a missed root/side branch or broken tip be obvious by
now. Since been blocked open in the chair throughout most of RCT I keep
finding myself clenching much more frequently, jaw exercises seems to help a
bit.
Any thoughts ?

Could be a missed fourth canal.

I would see your dentist again and have the tooth examined. It's possible
that there is another canal that was not seen, or it's possible that the
tooth is cracked, or there are other possibilities. After 3 months, a RCT
should be comfortable.

A possible guess would be the tooth next door, a missed canal, a cracked
root, or night-time clenching.
