What is the name of the procedure to pull out the middle of the face
(including upper jaw, nasal cavity. etc)? What kind of surgeon
(maxillofacial or craniofacial) should I see for a consultation?

Ditto, you *really* want an OMFS, and possible a consultation with an
orthodontist.

I believe you are referring to a Le Fort I osteotomy.
